ATHLETE INFORMATION FORMS are being submitted via Google Forms. Click on the apprpriate link below, fill out the form online, and hit the "Submit" button.

ALL OTHER REQUIRED FORMS

Certain forms are required before an athlete can begin participation in a sport. Please be sure ALL forms have been submitted before the first day of practice!

PLEASE NOTE: An athlete's physical form must be dated after April 15, 2019, to be accepted.

FOR SCIENCE HILL ATHLETES: We are using a website called DragonFly Max  to collect  and sign forms this year. You and your parents will need to CLICK HERE and follow directions of the pdf document

FOR LIBERTY BELL ATHLETES: We are doing it the old-fashioned way. Click on each of the links below to access the necessary forms.  Print them out, complete them and then give them to Coach Chris. The earlier you can complete this, the better.

SPORTS PARTICIPATION FEE
Both SHHS and LBMS charge a $25 Sports Participation Fee per athlete per sport.  This fee helps offset the cost of insurance for the athletic department.  Checks should be made out to your respective school and given to the coach.  In cases of financial hardship, see the Head Coach.
